鈍的外傷による眼球破裂およびその超音波検査の有用性について

Ultrasonography in ruptured eyeballs by blunt trauma

　鈍的外傷による眼球破裂4例に対し，受傷後早期より超音波検査を行い経過を観察した．direct ruptureと考えられる角膜・強膜破裂の1例は創部に網膜が脱出し，脈絡膜出血も強く術後眼球癆となった．他の3例はindirect ruptureによる結膜下強膜破裂で全例破裂創は赤道部を含み赤道部より前方にみられた．結膜下強膜破裂眼の受傷後早期の超音波検査の特徴的所見は，眼球の前後径の短縮像，眼球外の低エコー像，retrobul-bar spaceの拡大像などであった．これら結膜下強膜破裂3例のうち，破裂創が上鼻側輪部と下耳側前赤道部にみられ受傷後7日目に手術を行った1例は受傷後の期間が長く，眼球への侵襲が強く眼球癆となった．上直筋および外直筋の間の赤道部に破裂がみられた1例は術後増殖性硝子体網膜症を生じた．結膜下水晶体脱臼を伴い上方の輪部に破裂がみられた1例は術後矯正視力1.0を維持した．強力な鈍傷後に著明な低眼圧，浮腫を伴う結膜下出血，前房出血，深い前房などがみられる時は結膜下強膜破裂が疑われるが，さらにその診断には前記の超音波検査所見が極めて有用である．最後に結膜下強膜破裂の早期の診断とそれに対する早急な一次的修復手術の必要性を強調した．

We defined the ultrasonographic characteristics of eyeball rupture based on findings in 3 cases with rupture of subconjunctival sclera following blunt trauma. They included shortening of the axial length of the eye, low reflectivity induced by chemosii and hemorrhage in the anterior ocular segment , and enlargement of the retrobulbar space. Both A and B modes were useful. Use of ultrasonography would facilitate early diagnosis and treatment of eyeball rupture, which may frequently be hidden behind marked chemosis and subconjunctival hemorrhage.
