要約　目的：当院におけるT-fixation techniqueによる眼内レンズ（IOL）強膜内固定術の術後成績を後ろ向きに検討し，通常の囊内固定と成績を比較検討する。

対象と方法：岡山赤十字病院において，T-fixation techniqueでIOL強膜内固定術を行った22症例25眼を検討した。また，その僚眼に通常のIOL囊内固定が行うことができた13眼を比較対象とした。手術方法は，IOL支持部を25 G硝子体鑷子を用いて眼外へ抜き出し，強膜トンネル内に挿入した。

結果：視力は全例で改善し，logMAR視力平均0.59±0.56から0.22±0.39と有意であった（p＜0.05）。屈折誤差は−0.64±1.36であった。IOL傾斜は強膜内固定術で平均6.8±4.4°，囊内固定4.4±2.1°で有意差はなかったが（p＝0.09），IOL偏心は強膜内固定術で平均0.60±0.50mm，囊内固定は0.29±0.21mmと強膜内固定で有意に大きかった（p＜0.05）。術後合併症として，5眼（20％）に硝子体出血，3眼（12％）に低眼圧，1眼（4％）に一時的な高眼圧，1眼（4％）にIOL支持部の結膜下脱出がみられた。

結論：IOL強膜内固定術は僚眼のIOL囊内固定と比較して，IOL傾斜は有意差はなかったが，IOL偏心が有意に大きかった。今後は，屈折誤差や傾斜および偏心をさらに最小限にするべく，手術手技や固定位置の工夫などをしていくことが重要である。

Abstract　Purpose：To report and compare the outcomes of scleral fixation of posterior chamber（PC）intraocular lens（IOL）by a technique of T-fixation versus capsular fixation of PC IOL.

Subjects and Mtethods：25 eyes of 22 patients underwent intra-scleral fixation of PC IOL with a technique of T-fixation. Another series of 13 eyes that underwent capsular fixation of PC IOL were studied for comparison. Externalized haptics were placed into the scleral tunnel with a pair of 25-gauge vitrectomy grasping forceps.

Results：The mean logMAR of best-corrected visual acuity was 0.59±0.56. It improved to 0.22±0.39 at 6 months（p＜0.05）. The mean refractive difference from the predicted value −0.64±1.36 diopter. There was no significant difference with the mean IOL tilt between the intra-scleral fixated IOL group（6.8±4.4°）and the capsular fixated IOL group（4.4±2.1°）（p＝0.09）. The mean IOL decentration was higher in the intra-scleral fixated IOL group（0.60±0.50 mm）than in the capsular fixated IOL group（0.29±0.21 mm）（p＜0.05）. Postoperative complications included vitreous hemorrhage in 5 eyes（20％）, postoperative hypotony in 3 eyes（12％）, elevation of intraocular pressure in 1 eye（4％）, and IOL haptics fixed passed through scleral tunnel.

Conclusions：IOL decentration of intrascleral IOL fixation was higher than of capsular IOL fixation, but there was no difference with the IOL tilt. To minimize the IOL refractive difference, tilt and decentration, it is important to improve the surgical technique and fixate the haptics in better position.
