Abstract Purpose: To report a case of malignant optic glioma with favorable outcome following resection of tumor, radiation and chemotherapy.

Case: A 59-year-old woman was referred to us for suspected central retinal artery occlusion. She had noticed sudden visual loss in the left eye but failed to receive medical advice.

Findings and Clinical Course: Best-corrected visual acuity was 1.2 right and no light perception left. Left optic disc was pale and swollen with indistinct margin. Magnetic resonance imaging(MRI)showed swelling of left optic nerve anterior to the chiasm, suggesting optic nerve glioma. She received surgical removal of left eye and excision of left optic nerve. The removed specimen was diagnosed

Conclusion: Optic nerve glioma in an adult usually exacerbate after detection with poor prognosis. The present case illustrates that it may result in favorable outcome for at least 3 years after excision, radiation and chemotherapy.
