Abstract. Purpose:To report a case of giant epidermoid cyst in the orbital apex with optic atrophy as the initial clinical manifestation. Case:A 53-year-old female presented with gradually progressive proptosis and visual impairment in the left eye. She had suffered from blunt trauma in the left eye 12 years before. Findings:Corrected visual acuity was 0.6 right and no light perception left. Hertel's exophthalmometer showed 18 mm right and 29 mm left. The left eye showed advanced optic atrophy. Computed tomography showed an oval tumor measuring 50 mm near the orbital apex pressing the optic nerve. T2-weighted magnetic resonance imaging showed intense signal with no enhancement. The tumor was removed by frontal craniotomy and was diagnosed as epidermoid cyst. Conclusion:The orbital tumor in the present case was larger than reported in literature. Past blunt trauma may have contributed to progression of the tumor.
