Abstract. Purpose:To report a case of presumed optic neuropathy with angiitis related to anti-neutrophil cytoplasmic antibody(ANCA). Case:A 74-year-old female presented with impaired vision in the right eye. She had been diagnosed with uveitis in both eyes and been treated with peroral prednisolone. She had history of difficulty in hearing and cataract surgery in the right eye. Findings:Corrected visual acuity was 0.1 right and 1.2 left. The right eye showed signs of anterior uveitis and vitreous opacity. Fluorescein angiography showed findings of posterior uveitis. The left eye was apparently free of pathological findings. She showed elevated MPO-ANCA and was diagnosed with ANCA-related angiitis. Peroral prednisolone was continued. She noted impaired vision in the left eye and visited us 3 weeks later. Visual acuity was 0.3 right and no light perception left. Fluorescein angiography showed hypofluorescence in the left optic disc. She was diagnosed with presumed posterior ischemic optic neuropathy related to ANCA-related angiitis. The left vision failed to improve. Conclusion:The present case illustrates that optic neuropathy in ANCA angiitis may be intractable and may manifest variable findings.
