外傷性毛様体解離に対する8-0吸収糸を使用した強膜毛様体縫合術

Suturing of ciliary body to the sclera using absorbable 8-0 thread in a case of traumatic cyclodialysis

要約　目的：外傷性毛様体解離に対して，吸収性縫合糸で毛様体を強膜に縫着した症例の報告。症例：44歳男性が，軟式野球ボールで左眼を受傷し，その2日後に受診した。所見：矯正視力は右1.5，左1.0で，左眼が浅前房で，低眼圧であり，眼底に網膜下出血があった。16日後に左眼が遠視化し，低眼圧黄斑症の所見が生じ，視力が0.7になり，耳側120°の範囲に毛様体解離があった。受傷44日目に吸収性の8-0縫合糸で，毛様体を4箇所で強膜に縫着した。その1か月後に視力は1.2になり，眼圧が13mmHgに回復した。毛様体縫着による合併症はなかった。結論：吸収性縫合糸による毛様体の強膜への縫着は，有効で安全であった。

Abstract.　Purpose：To report a case of traumatic cyclodialysis treated by suturing of ciliary body to the sclera using an 8-0 absorbable thread. Case：A 44-year-old male was referred to us after being hit by a flying baseball in the left eye 2 days before. Findings：Corrected visual acuity was 1.5 right and 1.0 left. The left eye showed shallow anterior chamber, hypotony, and subretinal hemorrhage. On day 16, the left eye showed hyperopia, visual acuity of 0.7, and hypotony maculopathy. Gonioscopy showed cyclodialysis extending over 120 degrees of circumference in the temporal sector. On day 44, ciliary body was sutured to the sclera at 4 sites using absorbable 8-0 suture. Visual acuity improved to 1.0 one month later with intraocular pressure of 13 mmHg. There was no complication due to suturing. Conclusion：Suturing of ciliary body to the sclera using absorbable 8-0 thread was safe and effective.
