経強膜毛様体光凝固術の予俊解析と毛様体冷凍術との比較

Effect of cyclocryotherapy and transscleral laser cyclophotocoagulation in refractory glaucoma

　Nd：YAGレーザーによる経強膜毛様体光凝固術の術後成績を生命表で解析し，毛様体冷凍術と比較した。前者の対象は36例36眼で，経過観察期間は1ヵ月から21ヵ月，平均9.2ヵ月，後者の対象は80例80眼で，経過観察期間は2ヵ月から71ヵ月，平均21ヵ月であった。全経過におけるどの時点でも前者の累積生存率が，後者の累積生存率に比して高かったが，有意差があったのは，術後5ヵ月から10ヵ月の時点のみであった（P＜0.05）。合併症の頻度では，前者では5.6％に合併症を見たのに対し，後者では，20％になんらかの合併症を認めた。術後の疼痛は前者の方が後者より少ないと考えられた。

We evaluated the effects of transscleral lasercyclophotocoagulation (TSYLC) in 36 eyes withrefractory glaucoma as compared with anotherseries of 80 eyes treated by conventional cyclo-cryotherapy. The eyes treated by TSYLC consistedof primary open angle glaucoma 12 eyes, hemorr-hagic glaucoma 5 eyes, other secondary glaucomas15 eyes and congenital glaucoma 5 eyes. The eyestreated by cyclocryotherapy consisted of primaryopen angle glaucoma 3 eyes, primary angle closureglaucoma 5 eyes, hemorrhagic glaucoma 29 eyes,other secondary glaucomas 37 eyes and congenitalglaucoma 6 eyes. The postoperative follow up aver-aged 9.8 months in the former and 21 months in thelatter group.

We concluded TSYLC to be as effective as cyclo-cryotherapy in pressure-lowering effects and to besuperior due to fewer complications.
