Abstract. Purpose:To report a case of keratouveitis due to herpes simplex virus(HSV)with negative PCR test in the aqueous humor. Case:An 80-year-old female presented with pain, hyperemia and impaired vision in her right eye since 2 weeks before. She had had similar symptoms during episodes of common cold before. Findings:Corrected visual acuity was 0.03 right and 1.2 left. The right eye showed ciliary injection, hypopyon and corneal neovascularizations. She showed no eruptions in the eyelids nor dendriform corneal lesions. The aqueous proved to be negative for PCR test. She had another episode of fever and common cold 3 months later, followed by exacerbation of inflammatory signs in the aqueous. The right eye later developed dendriform lesions in the cornea, leading to the diagnosis of HSV keratouveitis. Topical treatment with acyclovir was followed by recovery. Conclusion:This case illustrates that initial PCR test of the aqueous may prove negative in HSV keratouveitis.
