「脳神経と眼科」佐野教授・鹿野教授の対談を拝見して（臨床眼科18巻12号）

Surgical cure of fracture of the optic canal. A refutation.

I．緒言

　いわゆる視神経管骨折を論ずるにはその全周を検査し，あるいは全周を対象として手術をしてその手術法の良否を論じなければならない。先ずこの対談は視神経管上壁のみを扱われる佐野教授のお考え，研究業績でわれわれの視神経管開放術を論じた所にまちがいがある。鹿野教授は視神経管上壁の圧迫のみで，われわれの多年の業績を論じていられるが，軽卒のそしりは免れない。従つてこの対談にはわれわれの行つている上眼窩裂減圧手術によつて，視神経と一緒に障害された動眼神経，外転神経麻痺の手術的療法は述べられてない。

　著者は昭和39年3月，東京眼科集談会の席上で鹿野教授の面前で20例の成績をあげて講演した。この対談のようなお考えならば，このような手術法をすれば，このような立派な成績があがるからと，症例，手術成績，遠隔成績をあげて，仁保，佐藤式手術を止めるように発言されては如何なものであろうか。

12 cases are described in detail in whom decidedlybeneficial effect could be brought about byremoval of fractured bones around the optic canal through transethmoidal route. Early intervention is essential in abtaining satisfactory results. It is recommended that surgery should be done within one week after trauma when the visual acuity is zero to 10cm/n.d. and within 3 weeks when the vision is less than 0.1. In cases with visual acuity of morethan 0.1, surgery is indicated when furtherdeterioration occurs in the visual acui-ty or the visual field.
