Clinical Application of the Kaolin-agglutination Test for Tuberculosis in Ocular Diseases

I．はじめに

　内因性葡葡膜炎の原因としては，以前は梅毒と結核が主流を占め，今日ではトキソプラスマ症，サルコイドーシスなどが重大な原因となり，梅毒はもはや影をひそめてしまつたが，結核に関しては，Woods^1）の統計からもうかがえるように，未だ全葡萄膜炎の3割を占めている。

　然し結核性葡萄膜炎は，他の原因による葡萄膜炎と同じく診断が非常に困難である。眼局所からの起炎菌の証明は眼球摘出の機会がない限り認めないから，通常我々は皮膚のツベルクリン反応，各種血清反応，房水の細胞反応，ツベルクリンによる局所病巣反応，肺結核病巣の既往歴，肺門リンパ腺の石灰化，抗結核剤の有効性（Diagnosisex juvantibus），他の原因除外（Diagnosis byexclusion）等を参考にして推定的に眼結核を診断している現状である。最近ではトキソプラスマ症を始めとして，サルコイドーシス，ヒストプラスマ症，ブルセラ症等の他の原因による内因性葡萄膜炎の存在が明かになつたことから，眼結核の診断はますます困難となつて来つつある。

One hundred and ninety-eight sera from patients with various ocular diseases and 148 sera from normal persons in Fukuoka city were examined for tuberculous antibodies by a kaolin-agglutination test using Takaha-shi's technique. The results were as follows :

1) Seventeen per cent of the 115 patients with retinochoroiditis showed possitive reac-tions to the titers of 32 or more.

In these cases of retinochoroiditis, only ca-ses of central exudative retinochoroiditis (Ri-eger) showed relatively higher incidence of positive reactions (27%) which was signific-antly higher than that for the normal popula-tion.
