One-quadrant argon laser trabeculoplasty

　隅角の1／4周に25発のレーザーを照射する1／4周argon laser trabeculoplasty （ALT）を原発開放隅角緑内障61眼，嚢性緑内障10眼，計71眼に対して行った。うち33眼では，2回目の1／4周照射を他の部位に追加した。術後経過観察期間は23から36ヵ月，平均28.2ヵ月であった。最終観察日に術前と同等以下の投薬内容で　20mmHg以下の眼圧コントロールが得られた症例は，原発開放隅角緑内障で33眼（54％），嚢性緑内障で8眼（80％）であった。生命表法（Kaplan-Meier）により求めた術後36ヵ月の眼圧コントロール率は，原発開放隅角緑内障で53％，嚢性緑内障で80％であった。これらの値は，先に行った1／2周ALTのものと有意差がなかった。また眼圧上昇に代表される術後合併症は，1／2周ALTのものに比べて頻度，程度ともに少なかった。1／4周ALTは1／2周ALTとほぼ同等の効果をもち，かつ合併症が少ないことから，より有用な治療法であると考えられた。

We applied argon laser trabeculoplasty (ALT), treating one quadrant of the angle circumference with 25 burns, to 61 eyes with primary open-angle glaucoma and to 10 eyes with capsular glaucoma. A second treatment over another quadrant was neces-sitated in 33 eyes. The follow-up period ranged from 23 to 36 months, the average being 28.2 months. We used the life-table method to analyze the overall success rate regarding the postoperative control of intraocular pressure below 21mmHgunder continued medication. We obtained the suc-cess rate of 53±6% for primary open-angle glau-coma and 80±7% for capsular glaucoma. These values were statistically similar to those for half -circumference ALT in our former studies. We noted the postoperative complications of the pres-ent method to be less frequent and less severe. One -quadrant ALT is to be preferred to half-circum-ference ALT on account of similar efficacy and greater safety.
