トラベクロトミーを併用した後房レンズ移植術の追跡

Results of combined surgery with trabeculotomy and intraocular lens implantation

　白内障，緑内障合併症例に対しtrabeculotomyを併用した水晶体嚢外摘出術，後房レンズ移植術（triple procedure）12例13眼を行い，術後7カ月から27カ月に及ぶ追跡を行った．本手術は，眼圧のコントロール，視野，視力予後は良好で，有効な手術であると考える．術後乱視も追跡最終時で平均0.88Dであった．しかしながら，末期視野障害症例に対する視力予後はあまり良好ではないという結果であった事より，中期までの緑内障症例に対して試みられる手技であると考えられた．

We treated 13 eyes with cataract and primary open angle glaucoma with a triple surgical proce-dure of trabeculotomy, extracapsular cataract extraction, and posterior chamber intraocular lens implantation. The follow up period ranged from 7 to 27 months.

Throughout the period of observation, the intraocular pressure was well controlled in all the cases. In early to middle stage glaucoma, we noted significant improvements in visual acuity and visual field when compared with our earlier series treated by cataract extraction and intraocular lens implantation alone. Throughout the cases, corneal astig-matism was negligible when assessed at the end of the followup period.

This triple procedure is judged as effective in controlling the intraocular pressure and in visual improvements, particularly in early and middle stage glaucoma cases.
