We evaluated 626 eyes in 332 cases with diabetic retinopathy using three-mirror lens and slitlamp microscope. We paid particular attention to the presence of posterior vitreous detachment and sim-ple or cystoid macular edema (ME or CME). Visual acuity and levels of hemoglobin A1c (HbA1c) were tested in all the cases.

Posterior vitreous detachment (PVD) was pre-sent in 231 eyes (36.9%), while 189 eyes (30.2%) did not manifest PVD or vitreous liquefaction. The posterior vitreous was incompletely detached in the remaining cases (32.6%). The incidence of ME and CME was significantly low in eyes with PVD than in those without (p<0.001). The prognosis of visual acuity was not correlatedwith the level of HbA,c. A certain number of patients with long history of diabetes manifested slow progression of retinopathy in spite of poor control of blood sugar. The majority of them had developed PVD.

Our findings show that PVD is closely related to ME and CME. The state of posterior vitreous plays an important role in diabetic maculopathy as well as in proliferative retinopathy.
