後部硝子体と糖尿病性網膜症（特に黄斑症）の関係について

Implication of posterior vitreous detachment in diabetic retinopathy and maculopathy

　長期にわたる糖尿病歴があり，糖尿病コントロール状態がきわめて不良であるにもかかわらず，網膜症がきわめて軽い症例を経験する．その様な症例には多くの場合後部硝子体剥離（PVD）が認められる．定期的にHbA₁cを測定している網膜症患者の後部硝子体と後極部網膜を観察し，単純黄斑部浮腫（ME），嚢胞様黄斑部浮腫（CME）とPVDが密接に関係しているという結果をえた．

　332症例626眼に対しHbA₁c，視力を測定し，Goldmann三面鏡細隙灯顕微鏡にてME,CMEおよびPVDの関係をしらべた．PVDおよび後部硝子体の液化を認めないものをPVD （‐）群，後部硝子体の分離（後部硝子体面が不連続なもの）を認めるものおよびPVD （‐）ともPVD （＋）とも判別のつかないものをPVD （±）群，後部硝子体の剥離（後部硝子体面が明瞭に連続したもの）または硝子体線維が認められない程の高度の硝子体液化を認めるものをPVD （＋）群と分類した．またMEおよびCMEを各々程度の差により軽いものより（‐），（±），（＋），（＋＋）の4段階に分類した．またHbA₁c値は8％未満のものをコントロール良群，8％以上10％未満のものをコントロール可群，10％以上のものをコントロール不可群とした．

　HbA₁c値と視力変化の間に相関はなく，PVD（‐）およびPVD （＋）各群でHbAlc値とMEおよびCMEの発生頻度に有意差は生じなかった．しかしPVD(+)群はPVD(-)群に比べてME(-)が有意に多く,ME(+)以上が有意に減少していた(P<0.001)．またCMEでも同様の傾向がみられ,PVD(+)群はPVD(-)群に比べてCME(-)が有意に多く,CME(±)およびCME(+)以上は有意に減少していた(P<0.001)．PVDの有無が増殖性網膜症だけでなく黄斑症発症にもきわめて重要であることが示唆された．

We evaluated 626 eyes in 332 cases with diabetic retinopathy using three-mirror lens and slitlamp microscope. We paid particular attention to the presence of posterior vitreous detachment and sim-ple or cystoid macular edema (ME or CME). Visual acuity and levels of hemoglobin A₁c (HbA₁c) were tested in all the cases.

Posterior vitreous detachment (PVD) was pre-sent in 231 eyes (36.9%), while 189 eyes (30.2%) did not manifest PVD or vitreous liquefaction. The posterior vitreous was incompletely detached in the remaining cases (32.6%). The incidence of ME and CME was significantly low in eyes with PVD than in those without (p<0.001). The prognosis of visual acuity was not correlatedwith the level of HbA,c. A certain number of patients with long history of diabetes manifested slow progression of retinopathy in spite of poor control of blood sugar. The majority of them had developed PVD.

Our findings show that PVD is closely related to ME and CME. The state of posterior vitreous plays an important role in diabetic maculopathy as well as in proliferative retinopathy.
