We evaluated the state of retinopathy in 46 insu-lin-dependent diabetics through 62 pregnancies.

In 27 subjects, retinopathy was absent during the initial stage of pregnancy. Mild retinopathy devel-oped during the course of pregnancy in 10 (37%), while retinopathy remained absent in the other 17. Nonproliferative retinopathy was detected in 35 subjects during the initial stage of pregnancy. The retinopathy exacerbated in 24 (69%), while retinopathy remained the same throughout the course of pregnancy in 11.

In the majority of cases, retinopathy developed or started to exacerbate at 25 weeks or later of gesta-tion. Cases with moderate retinopathy initially were prone to exacerbation. Retinal photocoagula-tion was effective in preventing further progression in these cases. Cases in whom retinopathy was absent in the initial stage failed to manifest retinopathy during pregnancy.

We identified risk factors for exacerbation of retinopathy during pregnancy : duration of diabetes longer than 5 years, poor metabolic con-trol during prepregnancy, and subjects of 30 years or older.
