免疫グロブリンの大量療法が著効したぶどう膜炎

High-dose intravenous gamma-globulin for acute uveitis

　左眼に，高度の網膜静脈炎とそれに伴う網膜浸出物，硝子体出血を伴った50歳の女性に，ステロイドの大量療法を施行したが無効であった．この症例に，免疫グロブリンを経静脈的に大量投与した所，静脈炎が鎮静化し，網膜浸出物，硝子体混濁とも著明に減少し，視力も0.02より0.6まで改善した．免疫グロブリンが著効した機序について，若干の考察を加えるとともに，免疫グロブリンの大量療法が，一部のぶどう膜炎に有効ではないかという事を示唆した．

A 50-year-old female presented with bilateral acute uveitis with retinal phlebitis, retinal exudates and vitreous hemorrhage. The disease continued to exacerbate in spite of massive peroral corticoster-oid therapy during the ensuing 3 months. We theninitiated a high-dose intravenous Human gamma-globulin therapy to be continued for 5 consecutive days. After two courses of the therapy, the retinal exudation and phlebitis rapidly subsided with dra-matic improvement in visual acuity.
