骨髄移植術後graft-vs-host diseaseの眼症状

Ocular involvement by graft-versus-host disease after bone marrow transplantation. Report of a case

　骨髄移植術後にみられた急性graft-vs-host病（GVHD）の1例について報告した．症例は急性骨髄性白血病の44歳女性である．3回目の寛解期にHLAの一致する症例の姉から提供をうけ，同種骨髄移植を行った．術後23日後に皮膚紅斑の出現と共に眼掻痒感，流涙，結膜点状出血が出現した．ひきつづいて結膜偽膜形成および偽膜の自然剥脱を認めた．偽膜の組織像はリンパ球，多核白血球を有する壊死性線維性膜像であり，炎症性にみられる偽膜像と同様なものであった．眼症状は抗生物質と人工涙液点眼のみで発症後45日には消失した．

　本症を含め現在までの眼GVHDに関する報告を基に考察を加え，本症における偽膜性結膜炎の発症には感染機序が関与したと考えた．

A 44-year-old woman with acute myelogenous leukemia of 2 and a half years' duration underwent allogenic bone marrow transplantation. She devel-oped acute graft versus host disease 23 days later, manifesting erythema of the skin, epiphora, conjunctival dot hemorrhages and eye itchness. Pseudomembranous conjunctivitis followed. His-tologically, the pseudomembranous tissue was com-posed of necrotizing fibrin membrane with lymphocytes and polymorphous neutrophils similar to those of inflammatory origin. Ocular symptoms disappeared 45 days after onset with topical instil-lation of antibiotics and artificial tear fluid. We concluded superposing infection caused or triggered the pseudomembranous conjunctivitis.
