眼球突出を来した蝶形骨fibrous dysplasiaの1例

A case of unilateral proptosis due to sphenoidal fibrous dysplasia

　右眼の充血および流涙のため受診した53歳の女性の患側に眼球突出があり，頭部レントゲン写真で蝶形骨の右大翼および小翼部に骨陰影の増強を認めた．視力は右0.3（0.8），左1.0（nc）．眼球突出度の左右差は3mm．右では中心フリッカー値の低下および視野狭窄があり，頭部CTおよび視束管撮影で右視神経管の狭窄を認めた．全身検査では異常はなく，蝶形骨のmonos-totic fibrous dysplasiaによる眼球突出と考え経過観察中である．

　Fibrous dysplasiaは一般に若年者の疾患と考えられているが，monostotic typeの場合は中高年者においても鑑別を要する．

A 53-year-old female presented with conjunctival hyperemia and epiphora in her right eye of 22 months' duration. The right eye was proptotic by 3 mm. X-ray and CT studies showed a tumor-like shadow in her right sphenoidal bone. The patient showed otherwise no systemic abnormalities. The finding was highly suggest-ive of monostotic fibrous dysplasia.
