慶大眼科における最近5年間の全層角膜移植手術例の検討

Results of penetrating keratoplasty in 99 consecutive cases duering a 5-year period

（1）最近5年間の慶大眼科における全層角膜移植手術99例の手術成功率は，術後1年目にて，角膜の透明度からみた場合80.8％，術後視力からみた場合69.7％であった．

（2）角膜白斑群，角膜ジストロフィー群，再手術群，水疱性角膜症群の4群に分けて検討したところ，角膜の透明度よりみた成功率はそれぞれ1年目1にて，87.2％，100％，53.8％，0％で再手術群，水疱性角膜症群が不良であった．

（3）手術不成功例19例の原疾患としては，角膜ヘルペスが6例32％を占めた．また不成功の原因としては，角膜内皮障害が，7例37％，拒否反応が6例32％を占めた．

（4）水晶体嚢外摘出術または水晶体嚢内摘出術の同時手術19例の1年後の角膜透明率は84.2％と良好であった．

We performed a total of 99 penetrating kerato-plasties in 90 eyes of 83 patients during the 5-year period ending 1982. The success rate was 81% in terms of clarity of the graft and 70% in terms of visual acuity.

The eyes were divided into 4 major types accor-ding to their presurgical state : corneal leucoma (39 eyes), corneal dystrophies including keratoconus (32), regraft (20) and bullous keratopathy (2). The success rate of surgery in each group was 87%, 100%, 54% and 0%, respectively.

The findings indicate that generally favorable results have been obtained in the present series.Recent advances in surgical technique and instru-mentation would allow improvements in regrafts and bullous keratopathy group.
