全層角膜移植の統計的観察—順天堂大学における最近24年間の症例について

A statistic study of penetrating keratoplasty.:A 24-year review

　1958年から1981年6月までに順天堂大学眼科学教室で行った全層角膜移植8171眼の術後混濁原因等につき検討を加えた。

（1）全層角膜移植術全体の透明治癒率は71％であった。

（2）角膜白斑例のうち，術前の内皮障害の著明なもの，混濁範囲の大きいものの結果は不良で，術後侵入血管の多少と透明治癒率との間に有意の関係は無かった。

（3）移植片反応は全体の27.4％に発症し，うち約30％が混濁結果に終わった。

（4）円錐角膜例における移植片反応は24.6％に発症し，その61％が術後3ヵ月までに，94％が術後1年までに起き，術後6ヵ月以上で起きた移植片反応では混濁結果に終わるものは少なかった。また角膜提供者の年齢が高くなるに従い，移植片反応出現率は箸明に上昇した。

We evaluated the results of penetrating kerato-plasty by us in 817 eyes during the past 24-year period ending June 1981.

Postsurgical clear grafts resulted in 71% of all the cases. Among the cases with corneal scarring prior to surgery, the surgical outcome was general-ly poor in eyes with severe and extensive endo-thelial lesion. There was no correlation between the postsurgical neovascularization and the visual outcome.

Postsurgical graft rejection occurred in 27.4% of the cases. About 30% of the cases with graft rejection resulted in permanent opacity.
