ペプシン処理免疫グロブリンの連日投与による実質型角膜ヘルペスの治療成績

Treatment of stromal herpetic keratitis with daily intravenous pepsin-treated human immunoglobulin

　実質角膜ヘルペスに対してペプシン処理免疫グロブリン製剤の点滴静注の投与方法が，その視力予後にどのような効果をもたらすかを検討した。

　当科を受診した実質型角膜ヘルペス18名19眼に対し，ペプシン処理免疫グロブリンを5H間連日投与（11例13眼），あるいはペプシン処理免疫グロブリンを1週間に1,2回間歇投与（11例13眼）した。

　ペプシン処理免疫グロブリンを5日間連日投与した群では，1週間に1,2回の間歇投与群に比べ，視力改善が著明であった。角膜実質浮腫および毛様充血に対しても，連日投与の方が効果は大きかった。前房内細胞および再発率には差が認められなかった。

　これらの成績より，ペプシン処理免疫グロブリンの連日投与（点滴静注）が，実質型角膜ヘルペスの治療に対して，間歇的投与より効果的であることが明らかとなった。

Eighteen cases with stromal herpetic keratitis (19 eyes) were treated by dripping intravenous infusion of pepsin-treated human immunoglobulin (2.5g). The treatment was given either for 5 consecutive days (13 eyes) or once or twice a week for a longer period (13 eyes).

The therapeutic outcome was more favorable in terms of improvement in visual acuity in the group treated for 5 consecutive days than in the group treated by intermittent infusion. An improvement in visual acuity set in in 1 to 2 weeks in the first group after initiation of treatment. The treatment was also effective for corneal stromal edema and ciliary injection.
