網膜静脈分枝閉塞症における晩期合併症と光凝固療法について

Indications of photocoagulation for prophylaxis of late complications in branch retinal vein occlusion

　網膜静脈分枝閉塞症の経過中に生じる硝子体出血，新生血管緑内障，網膜裂孔などのいわゆる晩期合併症を予防するために，光凝固の適応について検討した。合併症を認めた症例と認めなかった症例の螢光眼底血管造影所見を比較検討した結果，合併症を認めた症例には次の特徴が共通して認められた。

1）静脈閉塞部位が乳頭縁より1乳頭径以内に存在する。

2）無血管野が1象限以上におよぶ。

3）無血管野の動脈に著しい狭細化がみられる。

　よって，これらの条件を備えている症例に対しては，晩期合併症の予防のために無血管野全体に光凝固を施行するのが望ましいが，これ以外の症例では，新生血管やその他の合併症を認めた時点で光凝固を施行するのが妥当であると考えられた。

We reviewed a consecutive series of 16 eyes with severe late complications of branch retinal vein occlusion. The complications included massive and persistent vitreous hemorrhage necessitating vitreous surgery (7 eyes), neovascular glaucoma (4 eyes) and retinal tear formation (5 eyes). Fluorescein angiography was used in each case for evaluation.

All the affected eyes had the following charac-teristic features in common. The site of retinal vein occlusion was located within one disc diameter from the disc margin. The area of capillary nonperfusion involved a far wider fundus sector than one quan-drant.
