反復性眼圧上昇による視野障害の特性について

Evaluation of visual field change in recurrent intraocular pressure elevation

　Posner-Schlossman症候群8例11眼を対象に，反復する眼圧上昇発作時と寛解期における視野障害を，Friedmann M-IIを用いた静的中心視野検査による追究から，緑内障性視神経障害の障害機構解明へのアプローチを試み，次の結論を得た。

（1）反復する眼圧上昇発作による視野障害は，Bjerrum領域の中でも限局した感度低下野として出現し，発作の反復と共に，繰り返し同一部位に同様の感度低下野が再現される。

（2） Posner-Schlossman症候群における極初期の網膜神経線維層の障害は，Bjerrum領域のfine combed hair thinning （FCH）とraugh combed hair thinning （RCH）であり，これは場田のPosner-Schlossman症候群病期分類のI期以前に位置するものであり，その障害部位に一致し視野障害も進行する。

（3）眼圧上昇時および寛解期の乳頭の立体螢光眼底撮影からは，限局した部位の微小循環障害を示唆する所見は得られなかった。

　以上の結果は眼圧上昇によって起こる視神経障害が，lamina cribrosaのBjerrum領線維の通る部位の中でも極く限局した部位での直接的な障害により起こっていることを示すもので，特定血管の支配領域の障害とは異なる所見であった。

We evaluated the visual field changes in 11eyes with glaucomatocyclitic crises (Posner-Schlossman) during the recurrent crises and the remissions.Friedmann Visual Field Analyzer (Mark II) was used to estimate the visual field.

The visual field changes at the crises developed as isolated localized reduction in sensitivity along the Bjerrum area. These changes could be detected at the same site at each consequent crises in all the affected eyes. This small low-sensitivity area turned into fixed scotoma later.
