(18-P3-12) 上下半視野いずれかに局所性の視野障害を示す正常眼圧緑内障(NTG)17例17眼と正常人9例9眼を対象に,Heidelberg Retina Flowmeter (HRF)を用いて,乳頭周囲網膜組織血流量の測定を行った。正常眼の網膜上下の血流量には差がなかったが,NTG眼では,耳側上下,上下鼻側において視野障害側に一致する網膜は非障害側と比較し有意に血流が低下し(p<0.002),上下耳側ではFlow,Velocityでのみ認められた(p<0.028)。その血流量の低下と網膜感度低下量には有意な相関関係がみられた(p<0.05)。以上の結果より,NTG眼では視野障害進行と網膜組織血流障害に密接な関連があることが示唆された。
We measured the retinal blood flow in the parapapillary area in 17 eyes of 17 patients with normal-tension glaucoma (NTG) with localized visual field defect either in the superior or inferior visual field. Another series of 7 eyes of 7 healthy persons served as control. The measurement was made using a Heidelberg Retina Flowmeter. There was no difference in the blood flow volume between the superior and inferior parapapillary retina in the control group. In the NTG group, a significant decrease in blood flow volume was present in the area of visual field defect (p < 0.002). Similar decrease was present in blood flow and velocity only in superior or inferior temporal area. There was a significant correlation between blood flow and retinal sensitivity (p <0.05) . The findings show impaired circulation in the parapapillary area is closely associated with visual field defect in eyes with NTG.
