ハンフリー視野計による正常眼圧緑内障の長期臨床経過

Long-term course of normal-tension glaucoma evaluated by Humphrey perimetry

　正常眼圧緑内障と診断され，10年以上ハンフリー視野計で経過観察した13例26眼の長期臨床経過について報告した。初診時平均年齢は57歳6か月（42〜65歳），平均観察期間11年9か月であった。mean deviation（MD値），corrected pattern standard deviation（CPSD値）とも3dB以上の進行を悪化としたところ，全症例のうちMD値の悪化は6例10眼であり，CPSD値のそれは5例6眼であり，全体では9例13眼が悪化した。また26眼中22眼では矯正視力は1.0以上に保たれた。正常眼圧緑内障の進行，悪化はこれまでの報告のように全体的には極めて緩徐であった。しかしながら初診時から高度の視機能障害をきたしていた症例のなかに急激に悪化する症例がみられることから，症例ごとにきめの細かい対応が必要である。

We followed up 26 eyes of 13 patients of normal-tension glaucoma. The patients were aged from 42 to 65 years, average 57.5 years, at the start of follow-up. The duration of follow-up averaged 11.7 years. The visual field was evaluated using an Humphrey Visual Field Analyser. The visual field was judged as progressed when the mean deviation and/or corrected pattern standard deviation increased by 3 dB or more from the initial value. Out of the 26 eyes, the visual field defect progressed in 13 eyes of 9 cases. The visual acuity remained intact in 23 eyes during the follow-up. There was no difference in the progression of visual field defect between younger and older age groups. There was a tendency for advanced visual field defect to progress further, sug-gesting the need for a careful follow-up on case-to-case basis. The present findings show that deterioration of visual field in normal-tension glaucoma is much more gradual than hitherto reported.
