ベーチェット病における眼病変の変遷（第2報）

Changing pattern in ocular manifestations of Behçet's disease

　これまで我々はベーチェット病の眼病変の変遷について臨床的に検討し報告してきたが，今回は症例を増やしさらに四国地区の患者も加え，合計663名を対象として眼病変発症の年次によりA・B群にわけ，病型および性差，副症状の出現状態，眼病変発症年齢，視力予後について比較検討し次の結果をえた。

（1）病型では，B群における全症例および男性患者で完全型の有意の減少がみられた。男女比ではB群で男性患者が減少していたが，有意差はみられなかった。

（2）副症状の出現頻度は，全国調査結果とくらべ，眼症状を有する患者では消化器症状，血管系症状，神経系症状が有意に低かった。A・B両群の比較では，B群で関節炎症状，副睾丸炎，神経系症状が有意に低かった。

（3）副症状の出現数が2以上である症例は16.7％であった。

（4）眼病変発症年齢においては男女ともにA・B両群間に差はみられなかった。

（5）定型的な螢光眼底所見を示した網膜ぶどう膜炎型の視力予後は，発症後5年で判定するとB群の有意の改善がみられた。

We evaluated the symptomatology of 663 pa-tients with Behget's disease in order to ascertain the changes in clinical features of the disease. The pa-tients were classified into two groups : Group A, in which ocular lesions had set in before 1972, and Group B, in which ocular lesions developed after1973. Included were patients seen at Osaka Uni-versity since 1968 and at Tokushima University since 1981. Particular attention was paid to the revision in official diagnostic criteria for Behcet's disease adopted in 1982, in which the respiratory and urogenital symptoms were added to the minor symptoms of the disease.
