周辺性網膜剥離を伴う特異な網脈絡膜病変—第2報病因論の検討と鑑別診断

Retinochoroidal Affection Associated with Peripheral Retinal Detachment. Part II:Pathogenesis and Differential Diagnosis

緒言

　第1報^1）において，眼底後極部の灰白色性滲出斑によつてはじまり，その経過中に周辺性網膜剥離を呈した7症例の臨床像とその特徴について報告した。これらの症例は軽重の差，経過などで若干の差異をみとめるが，多くの共通点を示し，独立した疾患と考えられる。しかし，初発病変の性状，周辺性網膜剥離の出現，螢光眼底の所見など，個々の症状をとりあげてみると，類似する他の疾患も少なくない。本報では，本症に関する病因論的な検討と他の疾患との鑑別について報告する。

The occurrence of exudative maculopathy as-sociated later with peripheral retinal detach-ment was described elsewhere by the present authors (Fol. Ophthal. Jap. 24 : 130, 1973). The condition occurred only in middle-aged males, usually involved both eyes, started as multiple opaque exudative patches in the macula, deve-loped into peripheral detachment during its course, showed fluorescein effusion from the choroid corresponding in exudative patches, leaved scarring of the neuroepithelium of the retina, did not respond to systemic corticostcroid therapy and resulted in fairly well visual prog-nosis.
