Wagner症候群の一家系

A pedigree of Wagner's syndrome

　不完全ながら常染色体性優性遺伝を示すWagner症候群の一家系，3世代4症例について報告した。基本的な硝子体所見は，正常硝子体構造の破壊消失により，硝子体は光学的に虚の状態，硝子体索，網膜前膜，網膜硝子体癒着および後部硝子体剥離であり，網膜所見は種々な網膜変性，網膜裂孔，網膜剥離であった。更に眼内所見として白内障を認めた。これらの眼内所見は症例により様々な組合せで出現し，極めて多様性であった。本家系の症例中に，これまでに記載のない新しい網膜硝子体所見，すなわち硝子体中を浮遊する微細な硝子体線維の絡まりよりなる球状塊，紡錘状網膜変性巣，色素斑を伴う扇形の網膜変性巣および末梢網膜血管の走行・分岐異常が認められたことを指摘し，検討を加えた。Wagner症候群の診断は，その臨床像の多様性を十分に考慮し，家系調査結果をふまえてなされるべきであることを述べた。

We identified 4 cases of Wagner's syndrome in a pedigree involving 3 successive generations. The vitreous abnormalities in these case were charac-terized by the optical emptiness of the vitreous cavity, macrifibrillary degeneration, vitreoretinal adhesion, posterior vitreous detachment and forma-tion of preretinal membranes. The associated fundus lesions were nonspecific retinal degenera-tions and retinal breaks with or without retinal de-tachment. Cataract was found in 3 cases.
