悪性リンパ腫の治療中に発症した胞状網膜剥離の1症例—大量ステロイド剤による影響

Bullous retinal detachment associated with malignant melanoma and massive steroid therapy

　悪性リンパ腫の43歳男性にみられ，典型的な経過をとった胞状網膜剥離の1症例について報告した。本症の臨床および治療経過所見より胞状網膜剥離の発症三には，悪性リンパ腫の治療に使用した大臆ステロイド剤と上気道ウイルス感染がひき金になっているのではないかと推察した。また胞状網膜剥離は脈絡膜毛細血管板—Bruch膜—網膜色素上波レベルの機能低下により生ずる病態と考えられる点で，増田型中心性網脈絡膜症と共通の基盤にあると思われるが，増田型中心性網脈絡膜症の激症型とするより，一つの疾患単位と考えるのが妥当ではないかと考按した。

A 43-year-old male developed swelling in the neck since 6 months before and was diagnosed as malignant lymphoma. Bilateral bullous retinal detachment developed 8 days after initiation of systemic treatment with massive corticosteroid and cytostatics and 6 days after suspected upper respiratory viral infection. The eye conditions aggravated after a second upper respiraotry viral infection. The bullous detachment gradually resolved following cessation of corticosteroid admi-nistration 10 weeks later.
