難治性緑内障に対するpars plana vitrectomyを併用した毛様体解離術の成績

Cyclodialysis combined with pars plana vitrectomy for glaucoma

　虹彩切除trabeculectornyが無効に終った難治性の4例4眼の慢性閉塞隅角緑内障に対して毛様体解離術とpars plana vitrectomyを併用した術式を試み，3眼に良好な結果を得た。本術式は，過去の手術によって結膜が瘢痕化し，もはや炉過手術の効果が期待できないと考えられる症例にも有効であった。本法では，vitrectomyという過大な手術侵襲を行うから，本術式の適応はトラベクレクトミーなどの濾過手術が無効に終った閉塞隅角緑内障と広隅角緑内障に限定するのが適当であると考える。

We performed cyclodialysis combined with pars plana vitrectomy in 4 cases with chronic angle clo-sure glaucoma after peripheral iridectomy and tra-beculectomy had failed. After a follow-up period of 18 months and over, the intraocular pressure was controlled below 20 mmHg with topical pilocarpine alone in 3 of the 4 operated cases.

Our surgery was successful in eyes with conjunc-tival scarring due to previous operations and, there-fore, with poor outlook for another trabeculectomy. Because of the rather considerable surgical insult.
