眼内レンズ手術の増加と視力予後

The increase of intraocular lens implantation and prognosis of visual acuity

　1980年1月より1981年6月までに当教室で行われた白内障手術460例607眼につき，6カ月毎の3期に分け手術方法の推移，合併症，視力予後について検討した。

（1）当教室において近年全摘術が減少し，KPEを用いた嚢外摘出術が増加し，さらに人工水晶体挿入例が増加している。

（2）人工水晶体挿入例では，患者の平均年令は次第に高齢化の傾向にある。

（3）使用した人工水晶体は後房レンズが主流となってきており，3期では95％が後房レンズであった。

（4）人工水晶体挿入例の術中術後の合併症も全摘術と比較し，特に問題となるものはみられない。

（5）人工水晶体挿入例では術後早期より良好な視力が得られた。

　角膜内皮障害，人工水晶体の耐用性など種々な問題はあるものの，KPEと後房レンズ挿入の組み合わせは，将来増加して行くと考えられる。

During the past 18 months, 607 cases of cataract have been operated in our clinic. In the last 6 months, the cases of posterior chamber IOI, im-planted using KPE technique markedly increased. The complications of the IOL, implantation were equal to or less than the intracapsular extraction during and after surgery. The visual acuity was similar or better than in intracapsular extraction thoughout the postsurgical period.
