強度近視眼の眼内レンズ移植と希望屈折度

The preferred power of refraction after intraocular lens implantation for high myopia

　強度近視眼に適した白内障手術後の屈折度を知るために，白内障手術後の眼軸長27mm以上の症例84例121眼に対し，コンタクトレンズを用いて，屈折度を0D，−3D，−5Dとした時，好まれる屈折度について，アンケート調査を行った。術後矯正視力0.5以上の症例では，0Dと3Dの近視が好まれた。両眼の視力が0.1未満の症例では，近くに寄せることで字がみえるという理由から5Dの近視が好まれた。非手術眼が視力良好な有水晶体眼の症例でも，0Dと3Dの近視を好む症例が多く見られた。強度近視では個々の症例に対応した術後予想屈折度を選択することがより重要である。

We performed a survey by questionnaire in 84 cases of high myopia with cataract treated by intraocular lens implantation. After surgery, the cases were fitted with soft contact lenses of 3 refractive powers: 0,-3 and -5 diopters. In 65 cases, in whom the posteperative corrected visual acuity was 0.5 or better, the preferred refraction was in the order:-3D 48%, OD 38% and-5D 14%.In cases with visual acuity of less than 0.1,-5D was preferred by 80%, mainly because of ease of reading. Cases with unilateral phakia with good visual acuity tended to prefer OD or-3D.
