緑内障性視機能障害と色識別能—特にごく初期例について

Functional losses and color vision in ocular hypertension and early glaucoma

　緑内障にみられる色識別能力の異常が，高眼圧症の時期にすでにつかまえうるのか，それとも緑内障となって初めて出現するものか，初期の網膜神経線維層欠損のどの時期にどの様な異常がみられるかを検討するため，コントロールとして正常眼31例46眼，高眼圧症5例10眼および網膜神経線維層欠損の種々の程度の緑内障83例100眼で，Farns—worth-Munsell 100—Hue Testを施行した。

　その結果，色識別能力の異常は網膜神経線維層欠損が中等度以上に進行した湖崎分類Hb期以上で，青緑障害を主として必発し，網膜神経線維層欠損が進行するに従い青黄障害となる結果を得た。しかし高眼圧症や湖崎分類IIb期以前の緑内障では有意の異常はみられなかった。これはDranceらの高眼圧症眼で，青緑，青黄障害を伴う色覚障害を認める例は，将来視野障害を来たすという報告と相容れず，その原因についても検討した。

We evaluated the color vision impairments in 110 eys with ocular hypertension or glaucoma bymeans of Farnsworth-Munsell 100-Hue Test. The findings were compared with those in 46 normal eyes matched for age.

We found that the 100-Hue Test score was al-ways abnormally high in advanced glaucoma. It was normal in early glaucoma in which thin slit-like retinal nerve fiber layer defect (RNFL-D) is present with spot like scotoma in the Bjerrum area.

Color vision abnormality initially developed as apoor blue-green score and later as a poor blue-yellow score : a finding which contradicts the re-sults of Drance (1981).
