We made a retrospective evaluation of 151 eyes with acute angle-closure glaucoma during the past 15-year period, with particular attention to the clinical significance of secondary iris atrophy. The frequency of iris atrophy was higher in elder sub-jects : it was seen in 50% of cases below 59 years of age and in 100% in cases over 80 years.

The incidence of iris atrophy was correlated with the duration of acute glaucomatous attack : 29% when the attack persisted for less than 12 hours and 89% when it lasted over 24 hours. Patients with iris atrophy often showed optic nerve damage and low visual acuity.
