We evaluated the possible microcirculatory dis-turbances in the optic disc and the retinal area corresponding to the Bjerrum scotoma induced by elevation of intraocular pressure (I0P). Simultane-ous stereo-fluorescein angiography was performedin 7 glaucomatous eyes (7 cases) at normal and elevated IOP levels. All the eyes manifested no or minimum retinal nerve fiber layer (NFL) defects.

The dye-filling pattern in the optic disc was the same, qualitatively and gluantitatively, under the two IOP levels. Filling defects were noted in the bottom of glaucomatous cupping corresponding to the sector of NFL-defects. No tilling defects were found in eyes without NFL-defects or glaucomatous cupping.

Dye leakage in the disc region originated from racemose prelaminar capillaries and collector ven-ules. It either decreased or disappeared when the elevated 10P was reduced to normal value. Dye leakage was observed only after NFL defects be-came manifest.

The findings indicate that microcirculatory defect in the optic nerve head corresponding to the Bjer-rum area is not primarily caused by elevated lOp but as secondary to the NFL defects.
