Geographic helicoid peripapillary choroidopathy類似の1症例—網膜下新生血管の合併

Geographic helicoid peripapillary choroidopathy with subretinal neovascularization

　51歳の女性に発症したanterior uveitisとsubretinal neovascularizationとを伴ったgeographic helicoid peripapillary choroidopathyと思われる1例を報告した。anterioruveitisは右眼にのみ認められ，副腎皮質ホルモン点眼薬によく反応し，早期に消槌したことから，病変の活動期における一過性の反応性炎症ではないかと思われた。眼底病巣はステロイドホルモンを初めとする消炎療法に反応せず，本症を変性疾患とする説を裏付けた。subretinal neovascularizationは，脈絡膜変性に起因するBruch膜の破壊の結果生じたものと考えられるが，本症に合併することが少ないのは，本症における変性部脈絡膜血管の閉塞に基づくものと推定される。

A 51-year-old female developed geographic helicoid peripapillary choroidopathy in both her eyes. Anterior uveitis and subretinal neovascular membrane were associated with typical eyeground changes of this form of choroidopathy. Because anterior uveitis was well reactive to focal cortico-steroid therapy and short-lived, it might well be transient reactive inflammation of the active phase of this degenerative disease.

Fundus lesions were not noticeably altered by anti-inflammatory agents including corticosteroid. This fact favors a degeneration rather than an in-flammation.
