Geographic helicoid peripapillary choroidopathy （乳頭周囲地図状らせん状脈絡膜症）の症例

Geographic Helicoid Peripapillary Choroidopathy, A Case Report

緒　言

　眼底後極部に，散在性に斑状あるいは融合して地図状に網脈絡膜に浮腫様病変を来たし，あとに網脈絡膜萎縮巣を残す疾患がある。以前は，散在性脈絡膜炎とか黄斑部変性症と診断されていたが，最近，眼底検査法の進歩，ことに螢光眼底検査法の発達により，これらの病気のあるものは，脈絡膜または，網膜の滲出性炎症というよりは，網膜色素上皮細胞層を中心として，Bruch膜や脈絡膜毛細血管板に病変の主座があると推定される疾患が見出されてきた。その一つにGass^1）（1968）のいうacute posterior multifocal placoid pig-ment epitheliopathy　（A．P．M．P．P．E．）がある。GassのいうA．P．M．P．P．E．は，後極部に散在性に発症する斑状病巣で，急激に発病して，一時視力は悪化しても，その後に軽い変性を残すのみで視力予後は良好である。一方，それとは異なり，乳頭周囲に始まり，外方に向つて蛇行性に進行して黄斑部を侵し，あとに地図状の網脈絡膜萎縮を残して視力の永続的低下をもたらす疾患をMaum-enee^2）（1972），　Schatz, Maumenee　＆　Patz^3）（1974）は，geographic helicoid peripapillarychoroidopathyと名付けて，その9例を紹介し，一つのclinical entityをなす疾患であると報告した。

A 46-year-old Japanese male was found to have a bilateral, geographic helicoid peripapil-lary choroidopathy, so-termed by Schatz, Mau-menee & Patz in 1974. Ophthalmoscopic exami-nation of the right eye in which visual acuity had been lost for 20 years revealed geographic, widespread chorioretinal atrophy at the poste-rior region. Visual acuity of the left eye had been remarkably impaired for one week before the examination and marked edematous cloudi-ness of the deep layer of the retina at the papillomacular region was evident.
