Prophylactic measures against various complications of the KPE cataract operation

　Phacoemulsification （KPE）手術の最大の技術的問題点は，通常の手術器具の20ないし30倍も重く，しかも嵩張って扱いにくい超音波チップ（US tip）をいかに支え，いかに動かせば，眼球に力を加えずに思いどおりの動きができるかという事柄にある。

　術中の虹彩損傷は，術前極大に散瞳し，術中これを維持することにより，大部分避けることができる。

　術中の角膜内皮損傷は，水晶体が軟く手術時間が短くてすむものを選択すれば避けられる。

　後嚢の損傷は，注入・吸引チップ（I／A tip）使用時におこりやすいので，後嚢を吸引しないための注意が必要である。

　糖尿病患者の白内障手術にはKPEは不適当である。

Various complications of the KPE cataract ope-ration experienced in the first 10 cases were re-ported. They were prolapse of the iris during ope-ration, damages to the iris, to the corneal endo-thelium and to the posterior lens capsule and a postoperative hyphaema. Causes of these complica-tions were analysed and prophylactic measures pro-posed.
