Werner症候群白内障のOcutome／Fragmatomeによる手術

Use of Ocutome/Fragmatome for cataract with Werner syndrome

　（1）28歳男子のWerner症候群の1例を経験し，その両眼の白内障を，1眼は通常の水晶体全摘術で，1眼はOcutome／Fragmatomeによる小穿孔々超音波破砕法で手術し，結果を比較した。

　（2）通常の水晶体全摘術を行つた左眼は四面切開，厳重な縫合と意図的に創傷治癒の万全を期したにもかかわらず，術直後の前房出血，浅前房に始まり，半永久的な結膜下への前房水漏出等術創癒合不全による諸合併症を生じ，矯正視力は1.0に達しているものの未だに多くの不安を抱えて経過観察を行つている。

　（3） Ocutome／Fragmatomeによる小穿孔々超音波破砕手術を行つた右眼は術後1〜2週間，前房，硝子体中の蛋白・細胞増多，眼底小出血斑等を見たが術創部に全く不安がなく，術後矯正視力も1．0に達している。

（4）以上同一症例の両眼の手術結果の比較から，本症の白内障手術には小穿孔々超音波破砕手術が最もよい適応を有すると考えられた。

A male with Werner syndrome, 28 years of age, had cataract in both eyes. The left eye was operated on through conventional intracapsular extraction with four-plane incision and running 10-0 nylon suture. A persistent wound-leak had been observed after surgery although a 20/20 vision with correction was attained postopera-tively.

The right eye was operated on through pha-coemulsification using an Ocutome-Fragmatome System. There were no wound-leak postoperatively with a 20/20 vision with correction.
