Ocutome／Fragmatome systemによる硝子体・水晶体手術—その1 Ocutomeについて

緒　言

　硝子体吸引切除術は，硝子体出血などによる硝子体混濁の除去にはもちろん，各種前眼部手術たとえば硝子体の混じた外傷性白内障の除去などに極めて有用な手術法である。したがつて今日では硝子体吸引切除機は各種内眼手術にとつてなくてはならない機械器具となつている。硝子体切除機にもMachemerのVISCを始め種々のものがあるが，これらはカッターの駆動方式により二大別することができる。一つはハンドピース内にモーターあるいは電磁駆動装置を持つ「ハンドピース内電気駆動方式」のもので，VISC （Machemer）^1），Rotoextractor　（Douvas）^2），Vitreous stripper（Klöti）^3），　Vitreous nibbler　（Tolentino-Sche-pens）^4）などがこれに属す。他は「圧搾空気駆動方式」によもので，ハンドピース外に空気ポンプを有し，細いチューブで圧搾空気をハンドピース内のふいごの中に導きカッターを駆動するものである。例としては，ここにこれから述べようとするO'MalleyのOcutome^5）があり，またPey-manのVitrophage^6）がある。空気駆動方式のものは，一般にハンドピースの構造が簡単なので，煮沸消毒ができたり故障が少なかつたりいくつかの有利な特徴がある^7）。

Twenty-two eyes of 21 patients were operated on with Ocutome System. Nine eyes were of vitreous opacities mainly due to vitreous bleeding and eight eyes showed a marked improvement of their visual acuity after surgery. In other 13 eyes of 12 patients, anterior segment eye surgeries were performed using Ocutome System, because of traumatic in-juries to the lens, postoperative complications and others. In eleven out of these 13 eyes, Ocutomc showed a marked effect either improving the visual acuity of eliminating further complications.
