各種白内障手術の血液房水柵に及ぼす影響について

Fluorescein leakage through the blood aqueous barrier (BAB) after cataract surgery

　正常若年者7例，嚢内法（ICCE）群38例，嚢外法（ECCE）群32例，嚢外法および後房レンズ移植（PC-IOL）群23例を対象として術後血液房水柵（BAB）の経時的変化および術眼の僚眼における交感性反応出現の有無につきフルオロフォトメトリー静注法により検討した。10％フルオレセインナトリウム液1mg／kg体重静注30分後の前房内フルオレセイン濃度（Fa）と血漿限外濾過液中遊離フルオレセイン濃度（Fp）の比Fa／Fp （％）値をBAB透過性を表わす定量的指標として検討した。（1）術後3日，1週の時点ではICCE,PC-IOL群のFa／Fp値は25〜30％であり，ECCE群のFa／Fp値15％に比べ有意に高く，前記二術式においては，柵破壊の強いことが考えられた。（2）術後柵機能の経時変化は，ECCE群では1ヵ月以内，ICCE群，PC-IOL群では3ヵ月以内にFa／Fp値の術前レベルへの回復がみられ，修復されると考えられた。（3）各術式とも同一症例の僚眼におけるFa／Fp値は，術前，術後3日，1週でいずれも有意差を認めず，今回の検討では交感性反応を認めなかった。（4）術前のしかも白内障の存在しない健常老齢者群のFa／Fp値の平均は3.5％であり，健常若年老群の平均値1.9％に比べ有意に高く，老齢者群においてのBAB透過性の亢進が示唆された。

The disturbances of the blood-aqueous barrier (BAB) after various types of cataract surgery were evaluated by means of anterior segment fluoropho-tometry.

After thirty minutes following intravenous injec-tion of 10% fluorescein-Na solution (ling/kg), the blood was sampled to determine the free fluores-cein concentration in the plasma ultrafiltrate (Fp). Simultaneously, fluorophotometry was used tomeasure fluorescein concentration in the anterior chamber (Fa). We used the Fa/Fp ratio at 30 minutes as an index of permeability of the BAB.
