経強膜手術で治療した裂孔原性網膜剥離の長期予後

Long-term outcome of transscleral surgery for rhegmatogenous retinal detachment

（P−2-124）　経強膜手術で治療した裂孔原性網膜剥離の術後5年以上経過の追えた症例の長期予後を検討した。症例は144眼，術式は輪状締結術＋強膜内陥術85眼，強膜内陥術59眼で，研修医手術症例が89眼，指導医手術症例が55眼であった。研修医手術症例，指導医手術症例ともに長期経過後の視力は良好で，術後合併症の頻度も低く，術後長期予後に差はなかった。適切な指導のもとに行われる研修医の経強膜網膜剥離手術は有効で安全であると考えられた。また，黄斑剥離症例の71％が視力0.5以上に改善し，これらのうち64.3％は12か月以内に最高視力に達したが，35.7％は12か月以上たってから最高視力に達した。

We reviewed the outcome of transcleral surgery for rhegmatogenous retinal detachment in 144 eyes which could be followed up for 5 years or longer. Partial scleral buckling was performed in 59 eyes and scleral encircling with buckling in 85 eyes. The outcome was good regarding visual acuity or postoperative complications. There was no difference in the outcome or complications between 85 eyes operated by residents and 59 eyes by instructors. Among 42 eyes with macular detachment, visual acuity improved to 0.5 or more in 30 eyes (71%). Best visual acuity in the 42 eyes was attained within 12 months of surgery in 27 eyes (64%) and later in 15 eyes (36 %) .
