全眼球炎および眼内炎の20例

Panophthalmitis and endophthalmitis. Report of 20 cases

　全眼球炎および眼内炎の20症例を報告した。穿孔性眼外傷によるもの2例，白内障手術によるもの2例，結膜下注射によるもの1例，角膜潰瘍穿孔によるもの2例，緑内障手術後濾過瘢痕感染によるもの6例，転移性と思われるもの7例であった。

　患者側の素因として糖尿病が重要であり，20例中4例が糖尿病患者であった。

　濾過瘢痕感染6例のうち4例までがトラベクレクトミー後のものであった。トラベクレクトミーは従来の濾過手術と明らかに同じ危険性をもっている。

　起炎菌は16例中12例に検出され，以下のごときものであった。Staphylococcus aureus，Pseudomonas aeruginosa, Diplococcus pneumoniae, Streptococcus haemolyticus　（α—haemolytic type），　Streptococcus haemolyticus　（β—haemolytic type），　Enterobacter, Klebsiella aeurgenes．

Twenty cases of panophthalmitis and endo-phthalmitis during the past 14 years were evalu-ated. The condition occurred after penetrating in-jury (1 eye), cataract extraction (2), corneal ulcer (2), subconjunctival injection (1), as filtering bleb infection (6) and suspected metastatic in-fection (7). Diabetes mellitus seemed to be an im-portant contributing factor for endophthalmitis. It was present in 4 out of the 20 cases. Out of the 6 cases with filtering-bleb infection, four occurred after trabeculectomy. Trabeculectomy has therefore the same risk of late infection as the classical fit-lering surgeries.
