真菌性眼内炎の硝子体手術による治療

Vitreous surgery for endogenous fungal endophthalmitis

　内因性真菌性眼内炎10例16眼に対し硝子体手術を行った。7例に外科手術（喉頭部2例，腹部5例）の既往があり，全例に経中心静脈高カロリー輸液療法が留置されていた。治療の第1選択として，フルコナゾール，ミコナゾールまたはアンホテリシンBの抗真菌剤を使用したが，抗真菌剤に反応が悪いか，高度な硝子体混濁が生じた場合に硝子体手術を行った。術後視力は，2段階以上の改善が16眼中13眼（81％）に得られた。全例に眼内炎の再発を認めていない。硝子体手術は本症に対して有用な治療法であることが確認された。

We performed vitreous surgery in 16 eyes, 10 cases, with endogenous fungal endophthalmitis.Seven cases had been under intravenous hyperalimentation (IVH) following laryngeal or abdominal sur-geries. The endophthalmitis subsided in all the 16 eyes. The visual acuity improved by 2 lines or more in 13 eyes, 81%. The findings show the efficacy of vitreous surgery in cases of endogenous fungal endophthalmitis which do not respond to systemic antibiotics.
