A 69-year-old healthy female was operated for hypermature cataract in her right eye. The intra-ocular pressure was 29 mmHg OD and 10 mmHg OS. Crystalline floaters were present in the anterior chamber. Cataract surgery was done under local anesthesia with dichlorphenamide 150 mg as pre-medication. As the lens capsule ruptured, an un-planned extracapsular extraction was attempted. After delivery of the nucleus, a bulging of the iris occurred with loss of the vitreous. Anterior vitre-ctomy was done with full iridectomy. Suddenly, the retina and the choroid moved toward the wound.
