特異な螢光眼底造影所見を示した中心性漿液性網脈絡膜症の1例

An unusual clinical course of central serous chorioretinopathy

　36歳男性にみられた両眼性中心性漿液性網脈絡膜症の1例につき報告した。右眼の螢光眼底造影では，噴水拡散型→円型拡大型→下力漏出型→円型拡大型と漏出様式が変化し，下方漏出型になると共に漿液性剥離は下方周辺部に拡大し下液の移動性を伴った続発性網膜剥離の臨床像を示した。一方，左眼底では色素漏出点に一致してドーナツ型灰白色滲出斑の出現をみた。本症例の臨床経過より，CSCの螢光像および胞状網膜剥離の病因論につき考察を加えた。

A 36-year-old male developed bilateral central serous chorioretinopaty. In the right eye, the pat-tern of fluorescein leakage within the detached macula showed variable features during the course of 8 weeks. Initially, the dye diffusion was directed upwards in a smokestack pattern. At two weeks after onset, it turned into an inkblot pattern. At three weeks, the dilliision was directed downwards and the serous detachment extended towards the lower periphery accompanied by shifting of the subretinal fluid with change in eye position.
