アルゴンレーザーによる続発性開放隅角緑内障の一治験例

Argon laser treatment for a case of secondary open angle glaucoma

　Uveitisに続発した51歳男子の開放隅角緑内障で手術後，濾過孔が閉塞し眼圧32mmHgに上昇した症例にargon laserによる治療を行なった。Coherent社Model 900を使い，power 600〜650mW，照射時間0.2秒，beam-size200μ径にセットし，6時の部を中心に120°近くのtrabecularmeshworkにわたって38発の照射を行なった。術後1年以上にわたって眼圧は現在も20mmHg以下，平均19mmHgにコントロールされており，何らの合併症も認めていない。argonの照射条件，範囲等まだ検討すべき問題はあるが症例によっては有力な方法であると考える。

Application of argon laser to the trabecular meshwork successfully controlled secondary glau-coma in a 51-year-old male. The condition had started as uveitis with moderate elevation of in-traocular pressure in the right eye. The trabcular meshwork was heavily pigmented. Trabeculectomy was performed 14 months after initial occurrence of uveitis. Another 12 months later, elevation of intraocular pressure to 32 mmHg necessitated argon laser application to the trabecular meshwork.

Argon laser was applied to the trabeculum bet-ween 4 and 8 o'clock position.
