網膜剥離に対する強膜折込み術の一変法

A modification of lamellar scleral undermining for retinal detachment surgery

　網膜剥離に対する強膜折込み術の一変法について述べた。その術式は，従来の強膜短編ジアテルミー術とほぼ同じであるが，強膜への通糸法に工夫をこらした。すなわち両側に半層切開した強膜の一根部に通糸して内方に入り，同側根部から再び外に出す。この糸を対側の半層強膜へも同様に通糸し，ピンセットで両半層強膜を押し込みながら結紮すると，切開した強膜は全て眼球内へ折込まれる。本法で手術した裂孔原性網膜剥離は，29眼のうち27眼（93.1％）において網膜の復位が得られた。本法は，

　1）強膜半層切開の幅を自由に広げられる。

　2）強膜折込み量を加減できる。

　3）硝子体中に高い隆起が形成される。

　4）硝子体牽引を軽減できる。

　5）陥入物は自己のものである。

　等の特微があり，裂孔が1象限以内の網膜剥離に対しては，適切な方法と思われる。

Scleral buckling procedure for retinal detachment surgery employs silicone rubber or sponge which is foreign to the ocular tissue. We developed a modi-fied technique of scleral shortening-buckling proce-dure without having recourse to heterograft mate-rials.

According to our technique, after the sclera is incised to one-half of its thickness, transscleral dia-thermy coagulation and withdrawal of subretinal fluid are done. The sclera is then undermined on both sides of the line of incision to form flaps. The flaps are then bent inward to bulge and form scleral buckles by means of scleral sutures.
