前房隅角に白血病細胞の浸潤をみた続発性緑内障

Secondary glaucoma due to leukemic infiltration in the iris and trabecular structures

　症例は3ヵ月の男児で，睾丸腫大，腹部膨隆，チアノーゼなどが現われ本院へ紹介されて来た。腹部腫瘍の疑いで血液検査など精査の結果，急性骨髄性白血病と診断され，全身的に化学療法，放射線療法などが行われた。

　初診時より軽度の眼球突出が認められたが，2ヵ月の経過で左眼球突出が著明になり，左眼の眼圧に上昇がみられ，緑内障に対する手術を行った。

　組織学的検査では虹彩および線維柱帯網状組織に白血病細胞の浸潤がみられ網状組織は破壊され房水の流出障害を来たして二次的緑内障を起したものであった。また，視神経のくも膜も変性した白血病細胞の浸潤により軽度の肥厚がみられた。

A three-month-old boy who was admitted with enlargement of scrotum was diagnosed as acute myelogenous leukemia by peripheral blood exami-nation and bone marrow biopsy. He developed obstruction of the aqueous outflow channels as a result or leukemic infiltration of the iridis and ended with secondary glaucoma in the left eye, which is considered to be quite a rare occurrence.

Histological examinations showed the diffuse in-filtration of leukemic cells in the iris, trabecular meshwork and Schlemm's canal, by which the sinus structure was destroyed or partially dilatated.
