　網膜剥離手術4.5年後に，外傷の既往がなく，右眼に強膜および虹彩に断裂を来した症例を報告した。強角膜輪部から後方に大きな結膜嚢胞がみられた。従来の超音波検査では検出できなかったが，その嚢胞下にultrasound biomicroscope （UBM）にて強膜，虹彩の断裂がみられた。本症例は，UBMでなければ，強膜，虹彩断裂の検出は困難であり，UBMの有用性を示している。原因については，網膜剥離手術，あるいは肺癌手術後に投与されていた抗癌剤などの影響が関係したspontaneous intercalary　perforationの可能性があると考えられた。

A 55-year-old woman presented with a large conjunctival bleb in her right eye. She had undergone retinal detachment surgery in the right eye 4 years and 6 months before and 2 months earlier, surgery for lung carcinoma. Ultrasound biomicroscopy (UBM) showed ruptured sclera and the iris beneath the bleb. This feature could not be detected by conventional ultrasonography. The intraocular pressure was within normal levels throughout the course of observation. The scleral and iris rupture was interpreted as late complication of retinal detachment surgery in addition to chemotherapy for lung carcinoma.
