奇異な所見を呈した胞状網膜剥離の1例

Bullous retinal detachment with unusual fundus features

　47歳の女性に見られた胞状網膜剥離を報告した。まず左眼に発症し，4年4ヵ月を経て右眼に発症した。両眼共に早期の光凝固療法で視機能の低下は最小にとどまつた。右眼では後極部に巨大な馬蹄型病巣が出現し，螢光像では網脈絡膜関門の著しい破綻を示している。従来の知見からは網膜色素上皮細胞の変性が進行して巨大化したと推測されるが，我々は眼底像から色素上皮細胞間の離開を仮定した。いずれにしても特異な形態でかくも広範囲に網膜色素上皮層が障害されうることは特筆される。

A 43-year-old female developed bullous retinal detachment in her left eye. Xenon photocoagu-lation to areas of subretinal leakage and retinal pigment epithelial detachment resulted in clinical improvement till a similar lesion developed in the right eye 4 years and 4 months later. Nine days after photocoagulation to the right eye on a similar principle, a well-demarcated giant horseshoe area of opaque retina developed tem-poral to the right fovea. The area was character-ized by uniform, intense and increasing hyper-fluorescence.
